How to Obtain a Birth Certificate

The following individuals are eligible to request a birth certificate:

  • Registrant (the person named on the record), if 18 years or older.

  • Parent(s) listed on the birth record.

  • Legal guardian (must provide guardianship documentation).

  • Legal representative of one of the above (must provide supporting documents).

  • By Court Order (attach a copy of the court order).

If the registrant is deceased, a birth certificate marked “Deceased” can be issued to the following:

  • Spouse

  • Child

  • Grandchild

  • Sibling (if of legal age)

Instructions for Deceased Registrant Requests:

  • Note “Registrant Deceased” on the application.

  • Include a photocopy of the death certificate with the request.

Valid Photo Identification Required for Birth Record Requests

When requesting a birth record, you must provide valid photo identification.
